Support Coordination
At Xtramile Disability Care, we provide you with varying levels of coordination and transition support services. Xtramile Disability Care is a NDIS disability service provider in Adelaide, our services are planned to fit your individual needs and goals and take your circumstances and levels of complexity into consideration. We work with you and your family to build capacity so that we can work together to make you more independent.
There are three levels of support coordination
Support connection – This support service builds your ability to connect with informal, community and funded supports – Generally provided by the LACs
Support coordination – This support service includes a support coordinator who will work with you to ensure coordination of various supports to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and participate in your community.
Specialist support coordination – This is a higher level of support coordination for people whose situations are more complex and who need specialist support in getting rid of any barriers that may prevent access to your NDIS plan.
In-home support
If your disability or mental health condition impacts your ability to engage in household tasks, our experienced team of support workers can lend a hand. We know that having a clean and organized home promotes a sense of well-being and positivity.
Without interfering with your Independence and Personal Development Goals, we can assist with the following domestic activities.

Community Participation
Community participation is an integral part of the services we provide. We encourage your interests and focus on suitable programs that will connect you to your desired groups and services in the community.
We can assist with the following community services.
-Attending Personal Development Courses
-Work, Volunteering, and Mentoring to Overcome Challenges in the Workplace
-Participation in your desired Group Activities
-Participation in Social Groups
-Utilisation of Public Amenities like libraries, sporting clubs, movie theatres, banks, post offices, social events, etc.

-Liaising and connecting with Families, Friends, and Support Network
Specialised Mental Health Support with Gestalt Therapy
Because of dealing with a disability or being diagnosed with a mental health condition, many participants have a need for mental health care. The provision of personalised mental health care is one of our distinctive support services.
Our mental health approach is based on “Gestalt Therapy” which inspires participants to embrace personal responsibilities through the promotion of positive self-regulating and development strategies. We have been able to achieve significant success with a team of Support Workers with mental health backgrounds.
The participants will benefit in the following ways:
-Gain greater awareness of their situations.
-Improved ability to make informed decisions.
-Enhanced ability to recognise triggers and to respond more appropriately during crises
-Become more willing to accept personal responsibilities.
-Enhanced social interactions.
-Able to identify how their thoughts affect the quality of their life.
-Support with medication compliance and monitoring of desired, and side effects.
Supported Accommodation
Xtramile Disability Care provides a range of housing solutions to NDIS participants which are designed to maximise the independence of our clients within a caring and supportive environment. Our facilities are modern and purposely built to ensure complete satisfaction.
Depending on your needs and requirements, the following housing options are available to you:
Supported Independent Living [SIL]: refers to the support that is more commonly available in shared living arrangements. It includes services like having a support worker assist you with personal care tasks, or cooking meals. You are eligible to receive SIL support service if you live in a home with other people.
Independent Living Options [ILO]: refers to the type of service where you can choose the accommodation you prefer and the supports you will need. This two-stage service lets you explore and design the supports you need and is an alternative to SIL and to group home services. You can also choose to share your home with friends or housemates too.
Medium Term Accommodation [MTA]: is an intermediary support service that is provided to those participants that are awaiting a confirmed long-term accommodation vacancy. It can last up to 90 days.
Short Term Accommodation [STA] : is a support service that is funded for a short stay away from your usual accommodation. It is provided for up to 14 days at a time. You can choose to live by yourself or in a group setting.
Respite Care: is a support service that is available to you or your family when you need a break from your usual routine or in case of an emergency. It is time-limited and is available for a certain number of days in a year.
Specialist Support Coordination
Specialist Support Coordination refers to the time-limited support service that is made available to certain high-risk participants so that any barriers and complexities in their environments can be addressed. We help you connect with all the services and providers that support your goals. Xtramile Disability Care is a NDIS disability service provider in Adelaide.
Xtramile Disabilty Care employs highly skilled and dedicated Specialist Support Coordinators who manage and address all the challenges in your environment. The roles of a Support Coordinator and a Specialist Support Coordinator are very similar barring some key differences.
A Specialist Support Coordinator will be a qualified and experienced practitioner such as an Occupational Therapist, Psychologist or Social Worker. Their main aim is to reduce all barriers including health, education or justice service, that may prevent you from using or implementing your NDIS plan.

- Negotiate appropriate support solutions and assist you to access relevant supports during crucial situations

Therapeutic supports
Xtramile Disability Care offers therapeutic support services to help you improve your functional skills and independence. We assist with all regular and daily life activities like personal care, mobility, communication and socialising in the community. We also provide assistance and training so that you can build good, lasting relationships that help you move around in the community with ease.
Our dedicated team helps you to set goals based on your individual needs and help you achieve them. They may be provided in an individual or group setting, at home, another location or online. High-Intensive and Care & Community Nursing
We also specialise in the provision of High-Intensity personal care. We have support workers with the relevant qualifications and experience to deliver high-intensity support care. Our services are tailored to suit individual needs.
Some of the high-intensity daily personal supports include:
-Falls Prevention and dealing with emergencies
-Pressure Area Care and Prevention
-Required Activities of Daily Living – showering, toileting and pad change, general hygiene needs, assistance with dressing, etc.

Our Community Nursing Care is designed to support individuals with complex support needs that require the services of a registered or enrolled nurse in the community. The registered nurse will assess your clinical needs and draw up a suitable care plan in collaboration with you and your support network.
With our person-centered approach to care, you can be sure that your independence and general lifestyle will improve, and you will require fewer hospital visits.

Mental Health Care
Mental Health Care is a crucial support service that Xtramile Disability Care offers to participants. We understand and acknowledge the importance of providing good Mental Health Care services and thus have employed highly skilled and experienced team to provide supports with a goal to reducing hospital admissions for our participants.
The NDIS funds people with mental health conditions that have been diagnosed for up to six months. The funding includes supports that enables a person with a mental illness or psychiatric condition to effectively undertake activities of daily living, to participate in the community, in the workplace and in social circles.
Xtramile Disability Care will support you with information, medication management, motivation, counselling, care coordination, referrals and care escalations. You will also benefit either privately or in group from a special recovery programme called “Gestalt Recovery Pathway”.
Psychosocial Recovery Coaching and Mentoring
We have certified Psychosocial Recovery Specialists with qualifications and vast experience in mental health and disability care. We will work with you to identify your strengths and psychosocial goals. Then, with your lead, we will develop a plan that will facilitate your recovery in the identified areas.

Gestalt Recovery Pathways (Group & Private)
Gestalt therapy is one of the contemporary approaches to counselling and psychotherapy. It focuses on individuals’ present life and challenges (“the here-and-now”) and encourages positive self-regulating activities by promoting personal responsibility. The approach has achieved significant success in both individual and group counselling.
PURPOSE OF THIS COURSE
The main purpose of this course is to motivate clients to take personal responsibility over their lives and provide information on recovery pathways. It is the author’s hope that this course will enrich the participants’ coping techniques.
OUTCOME GOALS
- The participants will gain greater awareness into their situations
- Clients ability to make informed decisions will improve
- Clients will become more willing to accept responsibilities and respond appropriately during crises
- This course will enhance clients’ social interactions
- Participants will be able to identify how their thoughts affect the quality of their lives.
People with mental health issues like anxiety, depression, poor self esteem, self harm, suicide ideation and relationship difficulties will benefit from Gestalt Recovery Pathway.
Assist Travel / Transport
Using public transport to get to appointments, or to visit family and friends, can be daunting at times. Our dedicated staff work to increase your independence through public transport training. We aim to make your travel journey as smooth as possible so that you can go out and about into the community with confidence. We assist you to learn public transport routes, bus/train timetables, buying and recharging a Metro card, as well as how to use it.
Your NDIS plan may also include Taxi and Specialist transport arrangements if your disability prevents you from traveling independently.
Our transport services need to be booked in advance, and can help you get to:
- Medical appointments
- Place of education
- Work
- Local shopping centre
- Medical appointments
- Attending events, groups or clubs
- The park or library
Group Activities
At Xtramile, we provide a range of group activities that help you improve your wellbeing and make you feel included into the social fabric. We support you as you venture out into the community to get involved in local community groups, centres, events and clubs. We want you to feel confident by learning new skills and developing old ones.

Assistive Products for Household Tasks
XtraMile Disability Care believes in enhancing the independence, health and wellbeing of people with disability and our services are a reflection of our beliefs. We focus on supporting and encouraging our participants to lead a quality life. To achieve this we provide assistive products that are designed to ease daily household tasks of a participant.
Our expert team assesses your home and makes recommendations that can improve your daily routine around the house. Assistive products can be anything like a mechanical hoist to lift, a voice-enabled computer system, hearing aids, automated wheelchairs etc.
Our assistive products for household service include but not limited to:
- Mobility aids which aids in personal activity like showering, dressing/ undress toileting etc.

Life Skills Development
Our staff at Xtramile Disability Care provides support to help you develop life skills so that you can live independently within the community. We provide reliable and flexible support to suit your individual needs and goals. We want you to live as autonomously as possible and we can help you do that by helping you with new skills such as problem-solving, communication, social, daily activities etc.

Specialist Behaviour Supports
Xtramile Disability Care believes in supporting people living with disability in every way we can. Our support services are focussed on person-centred supports that will address the underlying causes of any concerning behaviour. Our program includes highly skilled Community Engagement Practitioners who assist participants to expand their opportunities for community participation and employment. Our approach focuses on capacity building and a gradual transition away from our service and eventually leads to informal supports and employment.
